Waiver and Release Form
The Party Event Waiver & Release Form is an agreement between a participant and ProConnect Persian Community Ltd. for an event. This Party Event Waiver & Release Form (hereinafter referred to as the “Agreement”) is entered into by the undersigned participant (hereinafter referred to as the "Participant") for the benefit of ProConnect Persian Community Ltd., their directors, officers, employees, agents, contractors, volunteers, sponsors, and affiliates (hereinafter collectively referred to as the "Company").

The Participant acknowledges, understands, and agrees as follows:

ASSUMPTION OF RISK: I am aware and understand that attendance and participation in the event (hereinafter referred to as the "Event") can be hazardous and involves a risk of physical injury or damage to property. I assume any and all risks, liabilities, and dangers associated with my presence and participation in the Event.

RELEASE AND WAIVER: I, on behalf of myself and my personal representatives, heirs, executors, administrators, agents, and assigns, hereby release, waive, and discharge the Company from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or equity, whether known or unknown, arising out of or in connection with any damages, injury, loss, or harm to me or to my property, or my death, while attending the Event.

INDEMNIFICATION: I agree to indemnify, defend, and hold the Company harmless from any and all claims, liabilities, damages, and expenses (including attorneys' fees and court costs) arising from or relating to my presence or participation in the Event.

PHOTO/VIDEO RELEASE: I grant the Company the right to photograph and/or videotape me and to use such photographs and/or videotapes for any purpose, including but not limited to promotional, advertising, and other commercial purposes.

GOVERNING LAW: This Agreement shall be governed by and construed in accordance with the laws of the state in which the Event is held.

REPRESENTATION: I am at least 18 years of age. I have read this Agreement and fully understand its terms and conditions.
